Job Description

Our employees work in engaged teams which are present in the lives of our residents where our employees make a difference.The OpportunityTemporary Part Time – 39 hours biweekly9 months with a start date of December 1, 2022 and an expected end date of September 12, 2023The Social Worker (SW) reports to the Manager, Corporate Best Practice/Education. The SW provides overall leadership, coordination and direction in the Social work interventions and care of the residents within the Mission, Vision and Values of Shepherd’s Care Foundation and standards of professional Social Work practice.KEY RESPONSIBILITIES

  • Assesses, plans, develops, implements, evaluates, communicates and documents all social work interventions in accordance with the Resident’s plan of care to achieve quality resident care;
  • Utilizes critical thinking, problem solving and decision making skills to provide competent, person centered care;
  • Participates in the development of plans, objectives and strategies for care services which are aligned with overall organizational strategic directions;
  • Participates in the development of care services and operational policies, procedures and programs in collaboration with appropriate team members, physicians, interdisciplinary team and other stakeholders;
  • Plans, develops, implements, and evaluates social work services in accordance with evidence-based practices;
  • Ensures social work practice complies with the organizational policies and procedures, continuing care health service standards, accommodation standards, relevant legislation and regulations, accreditation standards and regional health authority requirements;
  • Ensures completion of all Resident legal documentation as it relates to Guardianship, Trusteeship, and Powers of Attorney as required;
  • Responds, investigates and reviews concerns and complaints from residents, families and staff in accordance with the concern resolution process;
  • Acts as the Information Privacy Coordinator for Shepherd’s Care Foundation and complies with the relevant legislation;
  • Promotes educational opportunities, mentorship and leadership development for the staff;

The incumbent may perform other duties as assigned, duties not specified in this document.JOB SPECIFICATIONS/QUALIFICATIONS
  • A Social Worker Registration in good standing with the Alberta College of Social Workers (ACSW);
  • Bachelor Degree in Social Work preferred;
  • Minimum of two (2) years experience in geriatrics and continuing care preferred with;
  • Current valid driver’s license and driver’s abstract acceptable to the Employer to travel between SCF sites and committee involvement;
  • Commissioner for Oaths preferred;
  • Demonstrated skills in leadership, communication, teamwork, concern resolution process, customer service and time management;
  • Strong organizational, computer and presentation skills;
  • Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures.
