Job Description

We are officially launching our multidisciplinary interprofessional teams! It has never been a more EXCITING time to be a part of CMHA Peel Dufferin.The vision of redesigning our mental health and addiction regionally based teams was cultivated from the experience of having to adapt to the impacts of the COVID-19 pandemic. The pandemic created opportunities to innovate and reimagine services that align with the rapidly changing mental health and addictions needs of our community.Using the Quadruple Aim framework, we developed a stepped care model, which offers clients a wide range of interventions within a Multi-disciplinary Team. Clients and families will get access to the most appropriate interventions, with the right intensity, as quickly as possible. The development of the model was driven by assessment, using best practices and standards of care.Guiding Principles of Quadruple Aim:

These Multidisciplinary Teams (MDT) will be available across the Region of Peel and Dufferin and will include critical roles that deliver evidence-based services. Salary band is $52,753 to $63,158 annuallyWe are currently seeking a permanent full-time Short-Term Case Manager. The Short-Term Case Manager will provide time limited case management support based on needs assessments and care plans, to individuals experiencing serious mental illness and addictions concerns. They will work in collaboration with internal and external programs and agencies, while working within a holistic, integrated model of care to address clients unmet needs, using a stepped care approach.Reporting to the Multi-Disciplinary Team Manager, this position is based out of 10 Carlson Court, Etobicoke. We are currently operating in a hybrid work environment - flexibility to meet program and client needs is best suited to this role.What you will do
  • Act as initial entry to case management by conducting needs assessments, developing and implementing care plans with short-term objectives and ongoing monitoring until discharge
  • Provide short-term case management, task-based interventions and activities of daily living related to social determinants of health
  • Assess and provide crisis support and plans; make connection to crisis services when appropriate
  • Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed
  • Refer and link clients to community resources and groups that support recovery goals
  • Manage a caseload including regular re-assessments, updating risk levels and caseload reviews
  • Participate in regular staff meetings and care planning to provide case management expertise for client care within multi-disciplinary team
  • Facilitate groups as part of CMHA PD’s multidisciplinary team, including the creation of new groups and act as a back-up facilitator
  • Contact primary care at regular intervals to report clients’ care plan and discharge summary
  • Work collaboratively with other Case Managers to support organizational needs (e.g., volumes across all regions) covered by CMHA PD
  • Update all appropriate documentation within 24 hours of client contact, as required by CMHA PD policy and procedure
  • Participate in the Case Management Community of Practice and other agency and community committees and workgroups as requested by manager
What you will need to join our team
  • Post-secondary degree or diploma in relevant discipline (e.g. BSW, MSW, OT, RN)
  • Being a registered and/or regulated health professional is an asset
  • Minimum 2 years of experience with a focus on assessment, brief interventions and short term supports
  • Expertise in navigating community mental health and/or community based human services

  • Understand the experiences and needs of the mental health and addictions populations
  • Build rapport and initiate conversation with others using interpersonal skills
  • Approach client care non-judgmentally
  • Utilize trauma informed best practices
  • Demonstrate self-motivation to function as part of an interprofessional multidisciplinary team
  • Understand the importance of confidentiality and boundaries
  • Conduct assessments and coordinate services that are recovery-oriented based on need
  • Learn and understand community mental health and human service networks
  • Demonstrate crisis intervention skills for both telephone and in-person situations
  • Valid Canadian driver’s license and access to an insured automobile in good repair
  • A satisfactory Vulnerable Sector Screening (Police Check) report and an annual criminal records check (Police Check)
  • Proof of administered full-series Health Canada approved COVID-19 vaccination prior to start date. If unable to be vaccinated for reasons relating to protected grounds under the Code, completed medical or religious exemption must be provided
