Submit proof of COVID-19 immunization with your application.Reporting to the Supervisor Home and Community Care, the Home and Community Care Worker II is a member of the Home and Community Care team and is in frequent contact with the client and family. The HCCW II provides a key link to observe the client’s condition and any changes in their support network. The HCCW II reports any observed changes in the client’s condition to the Supervisor Home and Community Care or the Program Supervisor in the Community.The HCCW II ensures all client, family and program information, records and communication are treated with strict confidentiality; carries out the client care and/or service plans by following the pre-determined task list. This may include bathing, grooming, dressing, toileting, providing medication reminders and ambulation, including assistance with walking, climbing stairs and getting in and out of bed; performs specialized tasks or activities of daily living following individualized, client-specific training. These may include special care for palliative, rehabilitation and post-acute clients; prepares nourishing meals and special diets are required; plan meals with the client and assist client with grocery shopping; performs light housekeeping duties; assist in activities of daily living, other than personal care, where the client may have difficulties, such as watering plants, feeding pets, etc.; records data appropriately, accurately, completely and in a timely way to ensure clear and accurate client and employee records; and reports changes in planned hours of service to the Program Supervisor.The knowledge, skills and abilities are typically acquired through a completion of Grade 10 education and two (2) years of home care experience; an acceptable combination of education and experience may be consideredThis is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in more than one of Nunavut’s official languages would be considered an asset.
