You will convene and facilitate assessment and planning processes with individuals and/or families who have met the CYSN eligibility criteria and determine the supports and services that they require. Coordinating access to the best services available, you collaborate and build relationships with service providers, community partners, contracted agencies and foster parents. From an integrated service delivery system in the Ministry of Children and Family Development you may assist with placements, services for children, liaise and monitor contracts for services and ensure contract compliance and quality/appropriateness of services.This position requires strong administrative and organizational skills, the ability to work independently within a collaborative team setting and is comfortable working in a diverse culture environment. The ability to make decisions based on facts/assessments and build effective relationships with the clients is essential.
