Some collaborative practice options include Family Group Conferencing, Family Case Planning Conference, CareTeam, Youth Transition Conference, etc.The Collaborative Practices Facilitator’s role is to organize, coordinate and follow up on all elements of a conference and the resulting conference plan, including the orientation and preparation of the parents/youth and other family/community members.This position requires the ability to communicate effectively and being adept at developing and maintaining trust-based working relationships with a high degree of tact, diplomacy and respect for all parties involved.
