Job Description

Long-Term Care CoordinatorSTATUS:Full TimeLOCATION:Nursing DepartmentWAGE CLASSIFICATION:$29.89 – $34.67HOURS OF WORK PER PAY PERIOD:75 hoursPOSTING DATE:May 16, 2022EXPIRY DATE:May 23, 2022START DATE:As Soon As PossiblePURPOSE OF POSITIONThe Long-Term Care Coordinator plans, coordinates and organizes new admissions, care conferences and over quality of resident care.ACCOUNTABILITY

  • New Admissions
  • Daily Rounds
  • Family Council
  • Care Conferences
  • Oversee quality of care and performance management of team members
  • Follow up with residents and family compliance and concerns
  • Act as liaison with POA and/or Substitute decision maker
  • Other duties as assigned as per DOCs description
EXPERIENCE
  • Working experience with the elderly in a health care setting, preferably Long-Term Care and knowledgeable with respect to their needs
ABILITIES
  • Understanding Long-Term Care admission and discharge process
  • Knowledge of the Long-Term Care Act and regulations as well as Residents Bill of Rights
  • Understand complaint and concern process
  • Understanding Long-Term Care admission and discharge process
  • Effective customer service skills and a demonstrated commitment to working with seniors
  • Exceptional communication (both verbal and written) and customer service skills
  • Must be able to relate, communicate and co-operate effectively with staff, residents, residents families, and the public
  • Sensitive and empathetic to the needs of the residents.
