In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.Reporting to the Manager, Community Health Services, the Leader is responsible for the delivery of quality clinical care within the Community Health Services team including Case Managers; Home Care Nurses; Rehabilitation team members, Social Workers, Adult Day Programs. For certain locations this will also include other community based services including Urgent Care, Family Practice settings. The Leader works directly with the multidisciplinary staff as a leader, mentor, facilitator and coach to ensure that clients and families receive appropriate and excellent services. The Leader is involved in internal and external Community Health Services planning base on integrated regional services. This position is responsible for assessing, planning, implementing and evaluating care and practice activities at all levels at the area, team level. The Leader communicates all appropriate clinical, staffing and fiscal issue, along with proposed resolutions, to the Manager.QUALIFICATIONS:Education, Training And ExperienceA level of education, training and experience equivalent to a BSN or a degree in a relevant health discipline. Masters degree preferred. At least seven (7) years' health care experience including several years at the supervisory/management level.Skills And AbilitiesAbility to integrate administrative, professional and operational aspects of the program in an effective and efficient manner.Ability to develop an environment where quality improvement principles are embraced and used on a continuous basis.Ability to lead and manage, to motivate, delegate and operate in both structured and unstructured situations.Strong planning skills, strategic and operational.Ability to work with delegates to provide program services at multiple sites in a regional setting.Job Requirements:https://jd.viha.ca/JD4139.pdf
