Job Description

Qualification

  • Current registration and in good standing with the College of Nurses Ontario
  • Current CPRamp; First Aid Certificate
  • Proficient in the use of Microsoft Office
  • 3-5 years of leadership experience in the senior housing industry
  • Competency in the Retirement Homes Act and other applicable legislation
  • Familiarity with the provisions of the Occupational Health and Safety Act

Essential Duties, not limited to :
  • Recruitment and on-going performance management of all Wellness Staff
  • Participates in inter-professional assessments in identifying Residents needs and develops an individualized plan of care within 14 days of admission.
  • Attends Resident meetings as requested to discuss concerns/complaints, suggestions.
  • Assigns, delegates, and evaluates the activities of the wellness department.
  • Organizes and participates in the Resident Care conferences on admission, at 6-month intervals or if there is a change in the resident’s condition.
  • Ensure the maintenance of accurate health records.
  • Identify process improvements requirements and implement quality improvement as necessary.
  • Liaise with partners in hospitals, patient flow coordinators and other community referral sources.
  • Serve as the IPAC Coordinator for the community.
  • Completes all Wellness staff scheduling and adjustments as required.
  • Conduct home visits or on-site health assessments pre-admission to determine appropriateness for the community.
  • Participate in on-going resident review to assess the need for additional services and make recommendations.
  • On-going evaluation of Risk and remediation required to reduce the risk of injury to self, co-workers and residents.
  • Participates in fire safety demonstrations and fire drills.
