What is my role as Community Health Worker
  • Perform personal support tasks reflecting the needs of the client and family
  • Assist our patients with their activities of daily living tasks which may include:
  • assisting the client in walking,
  • climbing or descending stairs,
  • getting into and out of bed,
  • eating, dressing and other matters of personal hygiene.
  • Provide cognitive, social and/or emotional support to the client
  • Aid in personal care
  • Provides companionship and support
  • Light cleaning, preparation of nutritious meals, care of clothes, shopping for the client as required, and monitoring and maintaining household safety.
  • Provides simple bedside care, where required,
  • Monitoring and maintaining household safety.
  • Assesses the client's pattern of communication and ability to relate
