Social Prescribing Community Connector Seniors ServicesWho We Are:With a vision of a vibrant, connected community, Parkgate Society (PS) creates opportunities for people in our community to live life better. We support this vision through the development of a unique and diverse blend of accessible, low-cost, and no-cost programs and services, created in direct response to community needs; primarily serving children, youth, families, and seniors.Our dedicated and welcoming staff team, along with our many active volunteers, including our Board of Directors, are all committed to making Parkgate Society a welcoming and inclusive community services organization.Opportunity:The Seniors Community Connector will be the heartbeat of Parkgate Society’s Social Prescribing Program within our Seniors Services department. This individual’s outreach work will drive the day-to-day operations and associated programming for this service offering. The connector will receive referrals from North Shore medical sources, connect one-on-one with isolated and at-risk seniors needing support to find appropriate programs and services in their areas, and then present the needs to the associated community partners.This position will work as part of the team at Parkgate Society, which is physically located at Parkgate Community Center location and in the community and homes of the clients. The Seniors Community Connector will contribute to the overall effectiveness and success of our services for seniors, helping them to feel better connected to the community so they can live life better.Key Responsibilities:

  • Connect with medical and health professionals to receive senior participant referrals.
  • Interview referred seniors to determine their needs.
  • Source and attend seniors’ social programs to determine suitability.
  • Navigate and refer participants to local services and programs.
  • Monitor participants’ program effectiveness and adjust programming schedule as required.
  • Record program data and statistical information to maintain and complete reporting requirements.
  • Oversee volunteers’ placements, schedules, and performance.
  • Build and maintain relationships with program participants and address any questions, suggestions, concerns, and complaints as they arise.
  • Assist with the departmental advertising and promotion of seniors programming.
  • Participate in learning activities with other Community Connectors.
  • Perform other duties and responsibilities that may arise, based on the needs of the program.
Other Requirements:
  • Strong working knowledge and experienced background working in a not-for-profit, particularly in a community based and/or social service organization in programming.
  • Experience and working knowledge of available community resources and other not-for-profit organizations in the North Shore, specifically, would be an asset.
  • Specific previous experience working in or with seniors’ programming and services.
  • Good technical skills, including PowerPoint, are needed.
  • Strong ability to host effective presentations.
  • Strong interpersonal, relationship-building, and communication skills.
  • Demonstrated teamwork, facilitation, public speaking, and leadership skills.
  • Strong ability to adapt and pivot in a fast-paced environment.
  • Personal vehicle available for work, along with a currentamp; clear driver #39;s license.
  • Demonstrated ability to work independently, and as well as part of a team.
  • Ability to work effectively and build strong working relationships with a broad range of program staff, volunteers, not-for-profit or publicly funded groups, agencies, and organizations is critical for this role.
Contract Details:
  • We are located in beautiful North Vancouver, surrounded by forests, walking trails, and the ocean; Parkgate Society shares a physical space with Parkgate Community Centre, just up the hill from Deep Cove.
  • This is a part-time position, working 25–30 hours per week, Monday thru Friday.
  • Hourly wage is $25.00 – 28.00 per hour.
  • Three (3) weeks paid time off, employee benefits, and a professional development allowance is included, plus free parking.

  • Minimum 19 years of age.
