Job DescriptionThe Patient Care Manager (PCM), is responsible for providing and communicating strategic leadership and managing the day-to-day operations of the Childamp; Adolescent Psychiatric Inpatient Unit, Childamp; Adolescent Outpatient Services and Eating disorders Program for clinical and resource management.The PCM communicates and embodies the mission, vision, values of Halton Healthcare.Responsibilities encompass quality patient care, initiatives to improve care provided, promotes collaborative teamwork, patient satisfaction, professional and skill development of staff and other quality improvement initiatives aimed at delivering exemplary patient experiences.The PCM will ensure the safe and effective delivery of mental health services to patients admitted to the inpatient unit, Childamp; Adolescent Outpatient and Eating Disorders Program. This role involves managing a multidisciplinary team of approximately 66.5 full-time equivalent staff and works in collaboration with the Medical Director and 6 affiliated psychiatrists and 2 Pediatricians.Leadership: Provide strategic direction and operational planning for the Childamp; Adolescent Inpatient unit, Childamp; Adolescent Outpatient Services and Eating Disorders Program, aligning service delivery with organizational goals and community needs.Financial Oversight: Manage a budget of approximately $8M, ensuring effective financial planning and resource allocation in accordance with organizational priorities.Quality Improvement: Lead initiatives to enhance quality of care and patient outcomes, integrating evidence-based practices and industry standards into service delivery models.Collaborative Engagement: Collaborate with physician leaders, stakeholders, and community partners to identify clinical priorities and improve service delivery through patient and family-centered approaches.Partnership Development: Foster collaborative relationships with community partners to support integrated care.Leadership in Transformation: Lead efforts to transform the community hospital experience, setting client-centric goals and fostering a culture of continuous improvement within the Mental Health Program.This position reports to the Director, Mental Health and Addictions and contributes to the overarching strategic objectives of Halton Healthcare in delivering exceptional mental health and addictions care to the communities of Halton and Mississauga.
