Job Description

The Counsellor - Intake provides crisis intervention, system navigation, harm reduction, case management, outreach, information, support, and advocacy, referral, and short-term supportive counselling to clients of Carlington and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.Program-Related Activities

  • Engages in short-term assessment, referral, and advocacy activities with clients referred by Carlington staff and other area workers.
  • Acts as a resource to and advocate for individuals, groups, and the community.
  • Provides risk assessment, personal and practical support, information, referral and advocacy with individuals. Assesses client status by gaining an understanding of social determinants of health.
  • Acts as a client advocate on individual and systemic issues.
  • Able to provide addictions and mental health support to individuals and/or groups.
  • Provide crisis intervention services both over the telephone and in person.
  • Provides harm reduction services to clients at the centre and on an outreach basis as required. Acts as a liaison for the harm reduction workers in the community and community partner agency.
  • Contributes to chart documentation, participates in chart reviews and case conferences, reflective practice meetings/discussions and makes referrals.
  • Participates in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, families and the community.
  • Acts as a consultant on resources for other service providers in the Centre and in the community.
  • Liaises and collaborates with staff, teams and other community service providers as appropriate to provide service and make referrals.
