We provide a wide range of health care services such as primary health care, health promotion, community development, mental health and harm reduction. We make our work meaningful for our community by striving to ensure that our team reflect the community we serve.JOB SUMMARYWe work within an anti-racism, anti-oppression framework that acknowledges systemic racism exists and seeks to ensure that systems are fair and equitable for everyone.The Health Promoter conducts community needs assessments and coordinates, develops, implements, monitors and evaluates community development and health promotion initiatives in the SWCHC catchment area.The position will coordinate outreach, engagement and community building activities in the West Centretown neighborhood, with a focus on building a collaborative and inclusive approach to community safety and wellbeing. The Community Liaison and Neighbourhood Resilience project will feature outreach to businesses and residents as well as meetings/events designed to bring people together in a non-threatening setting to build trust and understanding, create opportunities for education and support, and collaborate to build a safer and more inclusive community.The ideal candidate will possess a clear understanding of issues of poverty, cultural sensitivity and harm reduction principles; is able to achieve a standard of excellence with work processes and outcomes; and requires a strong level of initiative and autonomy.AccountabilityThe Health Promoter is directly accountable to the Director, Family and Community Health & Harm Reduction and will work closely with other members of the Centre as required.POSITION REQUIREMENTSQualifications

  • Graduate degree from a recognized university in relevant discipline, or combination of undergraduate degree and related experience.
  • Experience in design, development and delivery of health promotion projects, using a community development approach
  • Strong relationship building and interpersonal skills
  • Experience working with individuals from differing social and cultural backgrounds
  • Demonstrated abilities in outreach and community engagement
  • Significant experience in program development, implementation, monitoring, and evaluation
  • Significant experience in project management
  • Significant experience working with marginalized populations, including people who use drugs
  • Significant experience in community engagement and mobilization
  • Significant experience in developing and maintaining partnerships

MAJOR RESPONSIBILITIESCommunity outreach, engagement and mobilization
  • Provide support to and seek input from the Centre’s Community Liaison Committee
  • Develop and implement a plan that provides outreach and community building activities in the West Centretown area to build connections between people who use drugs, local businesses and neighbours
  • Engages community in identifying issues and solutions while promoting community ownership of decisions
  • Identifies community priorities through relationship building, needs assessments, focus groups, and community forums
  • Provides opportunities for community building and community connections through training and events
  • Facilitates community participation/engagement/mobilization
  • Develops and implements community development and outreach strategies to reach community members with particular focus on residential and business neighbours and people who use drugs
  • Acts as a resource person and provides linkages with community members, groups, institutions, and businesses.
Project and resource management
  • Supports the development, implementation and evaluation of health promotion resources/materials
  • Identifies and develops strategies to support local initiatives. This may include writing and developing proposals, or supporting community groups to develop proposals that meet the needs of the community and within the Centre’s strategic plan and funder guidelines
  • Develops, implements, and evaluates projects and services and prepares funder reports as needed
  • Develops goals, objectives, and strategies of new projects
  • Provides ongoing management and monitoring of projects
  • Manages project budgets
  • Supervises students and volunteers as required
ORGANIZATIONAL RESPONSIBILITIES:
  • Contributes to the operationalizing the organization’s anti-racism and anti-oppression policy
