Job Description

POSITION: Ottawa East Ontario Health Team – Community Engagement LeadTYPE: Contract, Full Time to September 30, 2023SCHEDULE: 37.5 hours/week, Monday - FridaySALARY: $39.28-46.21LOCATION: Virtual/HybridHEALTH AT THE HEART OF THE COMMUNITY Ontario Health Teams (OHT) are a new way of organizing and delivering care that is more connected to patients, clients, families, and caregivers in their local communities. Under OHTs, health care providers (including hospitals, doctors and home and community care providers) and social service providers work as one coordinated team - no matter where they provide care. At maturity, each OHT will be responsible for delivering care for their patients/clients, directly connecting them to the different types of care they need and providing 24/7 help in navigating the health care system.Summary of responsibilities Reporting to the Population Health Manager, the community engagement specialist will be responsible for the following deliverables by September 30, 2023.1. Patient/client and stakeholder relationship management and supports

  • Coordinate and support the OE-OHT PCFAC, including scheduling and facilitating meetings, drafting agendas, creating slide decks, coordinating presenters, setting up meetings, administering compensation and recognition, drafting correspondence, and implementing evaluation.
  • Build capacity among client partners and primary care partners by developing tools and resources including manuals, handbooks, and policies that the OE-OHT can use for engagement, outreach, and education.
  • Develop, build and sustain meaningful relationships with diverse stakeholders representing the Ottawa East OHT communities.
2. Strategy
  • Undertake literature reviews, environmental scans on Best Practices in patient/client engagement.
  • Contribute to the design of local and inter-regional community engagement strategy and ensure strategic alignment with provincial direction
  • Engage with stakeholders to ensure diverse perspective and voice are included in all efforts.
3. Project Administration, Planning & Management
  • Provide project management leadership and support for the OHT’s PCFAC and its chairs.
  • Prepare and deliver reports, spreadsheets, letters, presentations, visualizations, and documents for OHT stakeholders as required
  • Assist in coordinating patient portal discovery activities and will work in collaboration with internal stakeholders, liaison with Ontario Health Teams (OHT) and Primary Care partners during project implementation.
Educations and Qualifications
  • Post-secondary degree/diploma in related field (e.g., health, education, volunteer management, social services)
  • Training and/or relevant professional experience in client and person-centred care, patient /client engagement, education, volunteer services, and/or client relations. Experience working with senior level executives and/or a diverse range of health service providers
  • Project management experience considered an asset
  • Demonstrated ability to build rapport and maintain relationships with diverse stakeholders (i.e., clients, family members and partners)
Knowledge & Experience * Extensive experience engaging with diverse clients, families and additional stakeholders across sectors to build relationships and improve collaborations.
  • General understanding of foundational OHT concepts including patient-centered care, health equity, collaboration, and collaborative decision making
  • Demonstrated understanding of the health sector, including primary care, acute care, home care, long-term care, and community support services
  • Understanding of change management principles
Abilities
  • Excellent organizational, time management and problem-solving skills
  • Skilled in the use of computers and relevant software applications (Microsoft Office – Teams, PowerPoint, Word, Excel, Planner, Virtual Meetings – Zoom, Microsoft Teams)
  • Strong observational skills and high attention to detail
  • Ability to work in a culture of continuous quality improvement
  • Excellent initiative and purposeful self-management skills
