We accept people without judgement and support them where they are at in their life.Job SummaryWorking closely with the Manager of Harm Reduction and the Overdose Prevention & Response Program Coordinator at Somerset West Community Health Centre and the designated supervisors at the Centretown and Carlington Community Health Centres, the Practice Facilitator will support and work alongside the Harm Reduction Peer Support Workers (HRPWs) as they provide services to people who use drugs in the Carlington, Centretown and Somerset West Community Health Centre catchment areas with a priority to engage with Black, Indigenous and Persons of Colour(BIPOC) Who Use Drugs.Hiring preference is given to qualified candidates who self-identify as Black, Indigenous or other Person of Colour.Position Responsibilities (includes but is not limited to):The Practice Facilitator will be responsible for working alongside Harm Reduction Peer Workers delivering outreach during evening and weekend hours. This will include providing real-time support and development to HRPWs in providing service delivery and increased system navigation for BIPOC People Who Use Drugs. Lastly, they’ll be responsible for overall project coordination including:

  • Establishing mechanisms to ensure the ongoing engagement and full participation of substance users in the planning, implementation and evaluation of the project; with a priority of increasing integration of culturally appropriate peer-based services
  • Organizing regular meetings and ongoing communication with partners
  • Ensuring that data collection and evaluation measures are in place
  • Working with project partners to develop a sustainability plan
  • All services are provided using a harm reduction model. Hours of work may vary depending on the need of the Program and will include evening/weekend work
Job Qualifications
  • Relevant combination of education AND/OR knowledge AND/OR experience equal to that of a Bachelor of Social Work (BSW) degree, OR a BSW
  • Lived experience of drug use an asset
  • Knowledge and experience with a harm reduction model of service delivery
  • Drug, sex and sex worker positive
  • Understanding of HIV and Hepatitis transmission
  • Knowledge of related community resources
  • Understanding of homelessness, mental health and substance use
  • Experience developing, delivering and evaluating training materials
  • Experience providing clinical supervision in a harm reduction setting
  • Experience supporting and supervising people who use drugs an asset
  • 3-5 Years experience providing direct service to people who use drugs from a Harm Reduction lens. Experience in Case Management and community outreach an asset.
  • Experience with Job Skills programs an asset.
  • Strong knowledge and ability in navigating issues related to clinical boundaries, professional ethics, confidentiality and relevant legislation including but not limited to PHIPPA and OHSA
  • Commitment to working within a Health Equity, Person-Centered and Anti-Oppressive Framework
  • Advanced knowledge and experience working with computers

  • Fluency in English
  • French and other languages an asset/preferred
Other Job Requirements
  • Needle exchange and safer inhalation training
  • Valid CPR
  • Ability to travel offsite on a weekly basis
  • Valid Drivers License and ability to drive Centre vehicles
  • Obtain Vulnerable Sector Screen Police Record Check for Executive Director review and sign-off. NOT required to be clear check.

  • Maintains and develops professional competence, and where applicable, a professional licence to practice, through appropriate continuing education methods (e.g. peer interaction, literature review, conferences, courses, staff development leave, etc.),
  • Complies and adheres to all applicable Centre policies and procedures,
  • Develops personal work plan in consultation with Manager, Community Engagement.

  • Contributes to the Centre’s work by participation in activities that seek to strengthen collaborative and interdisciplinary teamwork, such as intercomponent committees and working groups, strategic planning and accreditation.
  • Contributes to the Centre’s practices of hiring, orienting and training of employees
  • Participates actively in team meetings
  • Consults with and provides support to team members and other SWCHC employees regarding professional issues
  • Participates in the Centre’s strategic planning process
  • Participates in the development of policies and procedures by providing feedback in the Centre’s policy development process
  • Works in a manner that incorporates health promotion and recognizes the determinants of health
  • Participates in team and individual professional development opportunities
Occupational Health and SafetyThe Community Health Promoter must also work within the scope of all legislative and Centre policies related to occupational health and safety. In this capacity, the employee:
  • Must read and follow the Centre’s occupational health and safety policies.
  • Must have an OH&S Review at the beginning of their employment.
  • Must participate in any mandatory training the Centre provides including Worker or Supervisor Safety Awareness Training and WHMIS Training.
  • Must complete the AODA training
  • Must participate in Non Violence Crisis Intervention Training
