The selected candidate will be working with a clientele composed of elderly people in loss of autonomy. The job requires travel within the IUHSSC service territory.As a social worker within the Directorate Support Program for the Autonomy of Seniors (SAPA), you will ensure the best possible care for a clientele with a geriatric profile accompanied by a complex physical and psychosocial state of health, but with the potential to remain at home.Your interventions will be aimed at promoting the autonomy of individuals, supporting their ability to make decisions for themselves, maintaining their quality of life for as long as possible in their familiar environment and surroundings and facilitating access to the services to which they are entitled. Your role will be to ensure the continuity of services to the client and the cooperation of the members of the multidisciplinary team in the development and deployment of the intervention plan.Your duties will require you to :

  • Assess the clients psychosocial situation and needs by collecting and analyzing essential subjective and objective data;
  • Prioritize the clients needs, identify his or her disabilities, limitations and the limits of his or her network;
  • Proceed with the planning of appropriate services, taking into account the availability of resources;
  • Determine the partners to be mobilized (within the institution, within the network, in the community sector, etc.) and negotiate access to resources;
  • Intervene in the social functioning of residents, particularly in relation to interactions between them and their environment (living environment, family and loved ones, community) in order to ensure the optimal development of emotional, social and spiritual capacities and to maintain or improve quality of life and social functioning;
  • Coordinate the services put in place by ensuring optimal communication between the various partners involved (liaison officer role);
  • Monitor the services received by the client to ensure the continuity of interventions;
  • Carry out a clinical follow-up to verify that the objectives of the intervention plan have been reached and to readjust the plan, if necessary;
  • Carry out the necessary steps and accompaniment when a protection plan is put in place;
  • Perform the transversal functions of representing the clients interests and defending his rights;
  • Work within a multidisciplinary team with whom you will share the knowledge that distinguishes you and participate in team meetings.

Requirements :The desired candidate:
  • Must have a Bachelors or Masters degree in Social Work or a degree deemed equivalent by a competent authority*;
  • Must be registered as a social worker on the Roll of the Quebec Order of Social Workers and Marriage and Family Therapists;
  • Must be able to establish a therapeutic relationship with French and English speaking clients;
  • Must have experience with geriatric or home support clients;
  • Must have knowledge of issues and problems related to aging (cognitive disorders, dementia, bereavement, family dynamics, protective supervision, etc.);
  • Must have a valid drivers license and access to a vehicle for travel (mileage reimbursed);
  • Must have patience and empathy.
