Challenging work, welcoming communities, and expanded opportunities are some of the benefits offered to employees of the NTHSSA.Job InformationLocated in Sachs Harbour at the Tiktalik Health Health Centre, the incumbent reports administratively to the Nurse in Charge (NIC), and receives program support and direction from the Regional Manager, Community Health Centres. The incumbent provides culturally sensitive respite and ADL care to residents of Sachs Harbour. The NIC will identify the clients and accept them into the program based on a nursing assessment and the level of support or care required.Clients of the Home Care Program are usually individuals who require support and care so that they may remain living in their own home. These clients include but are not limited to long-term convalescent, geriatric, respite, surgical or medical patients who live within their home and require scheduled treatment, care and/or support. The clients medical outcomes are somewhat predictable.The Home Support Worker (HSW) is a member of the home care team who provides direct personalized care to residents who have common well-defined nursing diagnoses. The HSW will act as a clients advocate and facilitates communication between the client, family, Homecare Nurses, Registered Dietician, Occupational Therapist, Physiotherapists and other health care professionals.
