Your Opportunity:Working collaboratively as part of a multidisciplinary team, the Social Worker II - Case Manager is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Social Worker (Case Manager) is responsible for the provision of Social Work services to clients and families including psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, and making appropriate referrals to members of the multidisciplinary team. This position will be based out of the Strathcona Home Care office, but will provide support to home cares Suburban/Rural East team, which will include travel to the other home care network offices, as required.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
