CAREER OPPORTUNITY SOCIAL WORKER –Client Care Transition Worker– FULL TIMEAPPLICATIONS ARE INVITED FOR THE SERVICES OF A FULL-TIME SOCIAL WORKER – Client Care Transition Worker FOR THE ATHABASCA HEALTH AUTHORITY (AHA).POSITION SUMMARY: The Social Worker – Client Care Transition Worker will work closely with client care transition team under the supervision of the Director of Community Services or his/her designate. The Social Worker-Client Care Transition Worker will provide the necessary supports to clients under AHA care/on admission in any of our facilities with the provider scope of practice. The said admission could be residence such as but not limited to Safe Shelter, Respite Care, and/Inpatient.The Social Worker – Client Care Transition Worker will be responsible for Transition of Care activities such as but not limited to support for clients transitioning from Safe Shelter, Hospital and Long-Term care back to their respective community and family. This provider will refer clients necessary support services while in the community and working in collaboration with the care team. The worker will be responsible to perform all duties within the scope of practice of the Social Work profession and responsibilities that may be assigned to the worker by the Director of Community Services and/or AHA Senior Management.Primary Duties & Responsibilities: · Conduct a face to face and/or telephone assessment on complex care before clients being discharged from the Safe Shelter, hospital, supportive care and post-hospitalization.· Interacts with clients, family, provider(s) and coordinates the transitioning back to the community.· Execute planned non-clinical intervention(s) directed by licensed clinicians, which include but are not limited to facilitating timely access to services, ensuring there are no barriers to care and access to medication(s) and identifying assistance if needed on behalf of the clients and/or provider.· Assist clients with scheduling activities of daily living when necessary and/or specialist post-discharge follow-up appointments, securing transportation and other needs post-hospitalization or following discharge from the Safe Shelter.· Provides clients educational resources on healthy living and preventive health care measures.· Upon discharge from the Safe Shelter, hospital/inpatient services and/or specialized programs, the provider will have current client information, including the discharge instructions, discharge summary, current medication list or other information requested and provide necessary support on same.· Assist clients with outreach to community health centres and/or primary care providers as needed.· If necessary, refer the client to Health Coordination, Behavioral Health, and Disease Management and Pharmacy departments when additional assistance is required.· Maintains accurate and up-to-date written documentation and records activities in the information system according to appropriate clinical guidelines.· Ensures compliance with all provincial and federal guidelines, including HIPAA standards of confidentially of protected health information, reporting of critical incidents and reporting of quality of care issues.· Participates in staff meetings, interdisciplinary care team meetings and client-specific meetings as required.· Provide monthly, quarterly and annual reports on the activity update programs and provide necessary recommendations to optimize the functionality of the role and the program.Qualifications: · Bachelor degree in Social Work, Psychology or other Behavioral Science from a recognized program.· At least one (1) year of experience working in a clinical setting and/or in a managed care environment.· Ability to work with varied teams/professionals for inter-disciplinary case consultations.· Valid membership in a social work professional association.· Must possess strong written and verbal communication skills.· Knowledge of the Denesuline language would be considered an asset.· Valid Class 5 driver’s license· Clear Criminal Record and Vulnerable Sector Search.Individuals interested in applying for this position should forward their resume and 3 professional references including at least one from your current/previous employer to:Eileen MacDonaldDirector of Community ServicesAthabasca Health AuthorityP.O.
