Your Opportunity:Come join the dynamic Calgary Rural Pediatric Allied Health team! We are a strong, supportive team committed to sustaining a positive, respectful, proactive work environment. Working under the general supervision of the Calgary Rural Pediatric Allied Health Manager, the Pediatric Wellness Navigator functions as a key point of contact to assist families to way find and needed pediatric services. The Navigator within an interdisciplinary Allied Health team and supports referral intake and triage, acting as point person for families who are seeking support or information about health or community services for their child. The Navigator also supports families who are struggling with service coordination and helps ensure smooth transitions and warm hand-offs in care. Our child and family centred services focus on early intervention and child and family readiness. Interventions include assessment, diagnosis and treatment options that focus on functional needs that support meaningful participation at home and in the community; transitions through critical life and developmental stages and health promotion and injury prevention activities across the health system and community. The position will be situated in Strathmore but supports access to services in the communities of Strathmore, Didsbury and communities of the Bow Corridor (Canmore, Banff, Exshaw).Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
