LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient.To help fulfill its mission, LOFT is hiring a Case Manager.Job SummaryAs a Case Manager you will work to coordinate a support network for clients living with mental health challenges, substance use issues, physical health issues and experiencing homelessness. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect. Direct support will be provided to clients in service navigation, housing and shelter access, linkages to physical and mental health care, goal setting, crisis intervention, and advocacy.Positive Service Coordination (PSC) is a program within McEwan Case Management that provides short-term intensive Case Management for adults 16 or older living with HIV or AIDS and experiencing homelessness, mental health challenges and/or substance use issues. PSC also provides services to individuals who identify as trans, non-binary and gender fluid and require case management support.What You Will Do
