LOFT is also recognized as a Nonprofit Employer of Choice™ Award (NEOC) recipient.To help fulfill its mission, LOFT is hiring a Case Manager.Job SummaryAs a Case Manager you will work to coordinate a support network for clients living with mental health challenges, substance use issues, physical health issues, homelessness, and risks of housing loss. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect. Direct support will be provided to clients in service navigation and advocacy, linkages to physical and mental health care, goal setting, crisis intervention, and housing and shelter access.The Addictions Supportive Housing (ASH) team provides Case Management for adults 16 or older living with HIV or AIDS who face barriers related to substance use and mental health in the Toronto core. The position supports health care and system navigation, ensuring inclusiveness of 2SLGBTQ+ and BIPOC communities, with the goal of community re-integration and personal empowerment.What You Will Do
