Applicants are encouraged to self-declare in their application.Position SummaryConsistent with the principles of primary health and the Mission, Vision and Values of Mount Carmel Clinic, the incumbent works collaboratively with all Mount Carmel Clinic programs, community and hospital based hepatology clinics. The community hepatology nurse assesses, plans, implements, evaluates and coordinates care relating to Hepatitis C and generalized liver disease, education and support for clients in the community based nurse managed community hepatology clinic. The incumbent adopts a long term client centered approach to care consistent with the philosophy or harm reduction and as a means of ensuring continuity of care.QualificationsEducationamp; Licensing

  • Must be a member in good standing with CRNM
  • Must be licensed to practice in Manitoba
  • Must have experience taking responsibility for the activities of a nursing unit or program.
  • Must be a member in good standing with the Canadian Association of Hepatology Nurses, or will become a member
  • BSN preferred (RN with appropriate experience may be considered)
  • CPR certification at the Basic Rescuer Level
Skills, Expertise, and Experience
  • Minimum of 5 years #39; experience working in a community-based setting preferred.
  • Demonstrated expertise in Hepatitis C, generalized care, and non-infectious disease.
  • Provides HCV and lifestyle education to all hepatitis C clients, their significant others, peers, and community members, as needed
  • Provides hepatitis screening, HIV testing, HCV, and healthy lifestyle education to at-risk clients
  • Ability to work independently and in a nonjudgmental manner, within an inter-disciplinary team environment with respect, professionalism, cooperation, sensitivity, and cultural awareness
  • Excellent interpersonal, organizational, and communication skills (writtenamp; verbal) are essential
  • Collects, prepares, and submits reports and statistical information as required.
  • Acquires, develops, and evaluates teaching materials and tools with consideration for cultural, language, physical, intellectual, and environmental factors.
  • Participation on internal and external committees as required.
  • Participation, planning, implementing, and evaluation of clinic efficiency initiatives.
  • Participation and facilitation of education for staff, clients, and community.
  • Maintain up-to-date knowledge of emerging trends and issues relating to nursing, hepatological care, and chronic disease management.
  • Maintain up-to-date knowledge of guidelines and best practices for competent care
  • Ability to work independently and within an inter-disciplinary team environment with respect, professionalism, cooperation, sensitivity, and cultural awareness
  • Ability to work in a pro-choice community health facility in an inner city community and provide care using harm reduction principles
  • Ability to work within the impacts of the social determinants of health in an effective and non-judgmental way
  • Understanding of how culture and diversity influence the client service plan
  • Ability to mentor, support, and work alongside students and volunteers
