Job Description

Company BioHome and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement.These organizations were previously known as Local Health Integration Networks (or LHINs ) at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smith Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.Position SummaryReporting to the Director, this position is responsible for ensuring appropriate assessment of needs and coordination/management/provision of services. The position manages day-to-day operations to ensure that effective and efficient services are provided to patients and their families through the provision of case management and long-term care home placement services.Description of Specific DutiesPatient Service Delivery

  • Manages the delivery and/or access to coordinated, quality community health care;
  • Acts as the primary contact for patient/family complaints or MPP concerns on issues not resolved by Care Coordinator or team member;
  • Participates, supports and/or leads in planning, implementing, managing, and evaluating patient service delivery programs;
  • Monitors and reviews caseload statistics and indicators for assigned portfolio and team;
  • Oversee the utilization of purchased patient services for the organization.
Relationship Management
  • Builds and maintains effective relationships with health and social service agencies and external service providers to partner in delivering quality patient care and promote an integrated approach;
  • Represents Home and Community Care Support Services South East on multi-disciplinary committees and community agency/project working groups.
Management of Human Resources and Leadership
  • Manages overall performance of local care community team; provides direction on projects and priorities; orients and trains staff to equip them with the necessary skills needed to resolve issues and provide quality patient care; guides Care Coordinators in handling critical issues;
  • Identifies staffing requirements and recommends recruitment and selection of candidates ensure effective staff allocation within assigned budget.
Minimum Qualifications & Position Requirements
  • University degree preferred in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work;
  • Holds a current unrestricted registration as a member of the associated College;
  • 5 years progressively responsible experience in health care;
  • Management experience in a unionized environment with direct experience with change management preferred.
Patient Services
  • Experience and knowledge of service delivery in a health care environment;
  • Knowledge of direct care/case management models used in community health care organizations;
  • Good knowledge of community resources (e.g., services and programs), and roles of health care professionals;
  • Knowledge of challenges and issues, methods and techniques for outsourced/ contracted services and service providers;
  • Knowledge of tools, systems and databases used in patient service delivery and management.
HOME AND COMMUNITY CARE SUPPORT SERVICES /Industry
  • Knowledge of Home and Community Care Support Services South East business strategies, objectives, priorities and programs, and related Patient Services priorities and plans;
  • Knowledge of the evolving role of Home and Community Care Support Services South East and the issues and priorities within the healthcare sector and how these impact patient service delivery;
  • Practical knowledge of relevant legislation (e.g., Long-Term Care Act, Nursing Homes Act, etc.);
  • Knowledge of Home and Community Care Support Services South East administrative practices and procedures.
Administrative
  • Strong planning and organizing skills;
  • Ability to use MS Office applications (e.g., Word, Excel, PowerPoint, etc.).

  • Effective communication skills to maintain a range of contacts with health professionals, service providers, regulatory colleges, educational institutions/Boards, and other organizations within the community;
  • Strong collaboration skills to participate on projects and committees with colleagues across Home and Community Care Support Services South East or local community agencies;
  • Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings;
  • Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action;
  • Good coaching and communication skills to provide instructions and guidance to staff with respect to activities, challenges and questions.
Physical, Visual, Auditory and Concentration DemandsPhysical Demands
  • Work is generally completed at a desk or conference table;
  • Long periods of sitting and regular computer/ phone use;
  • The incumbent has freedom to move about or change position at will.
Visual Demands
  • Reads a variety of reports, including charts, statistics, slides on a routine basis;
  • Required to view a computer screen daily for responding to e-mail, preparing documents and reports.
Auditory Demands
  • Position functions in a normal office environment (e.g., uses the phone on a daily basis, etc.).
Concentration Demands
  • Absorbs and analyzes complex data and performance and management reports;
  • Required to listen to and reconcile multiple points of view being discussed/ presented;
  • Ability to read and analyze complex health related information and reports in order to develop responses to patient needs;
  • Interruptions are common and demands are made from staff, patient/family, partners and Management.
Problem Solving/ Complexity
  • As the first-level of management in a community area, this position is required to resolve problems and conflicts regarding clinical and interpersonal issues that require consideration of multiple sources of information;
  • Analyzes information from information systems/databases, service providers, patient reports, staff feedback and industry trends and practices to formulate recommendations and provide input to planning processes for future programs, policies and services;
  • Manages multiple priorities as circumstances and information change frequently.
Accountability & Decision-Making
  • Responsible for ensuring the effective delivery of patient services and operational support for a given portfolio;
  • Position is first-level management response at the local level and has authority to make decisions within established guidelines to address and resolve issues and challenges;
  • Responsible for staying informed of new developments and emerging issues in portfolio and providing information to the Senior Manager and to staff to take appropriate action.
Risk
  • Decisions and actions will have a direct impact on the quality of patient care within the designated portfolio and could influence public perception of and confidence in Home and Community Care Support Services South East;
  • Requests to release personal information from patients, lawyers, other public health agencies, WSIB, courts, etc. must be analyzed to mitigate legal and financial risks.
People (Leadership)
  • Manages the performance of direct reports;
  • Acts as technical mentor within and outside Home and Community Care Support Services South East;
  • Organizes, coordinates and reviews the work of colleagues, contractors/suppliers/service providers.
Health and Safety
  • Follows safe work procedures;
  • Reports injuries and illnesses;
  • Plans and carries out both work internally and externally in a safe manner;
  • Abides by all legislative requirements, and related policies and procedures;
  • Participates in related training and development, e.g. WHMIS;
  • Maintains competency related to organization’s Emergency Response Plan, e.g. participates in planned tests;
  • Reports unsafe or unhealthy working conditions;
  • Is a ‘competent person’ as defined by the Occupational Health and Safety Act, and responsible for duties as outlined in the Occupational Health and Safety Act in section 27 (1).
Working Conditions
  • Normal office conditions;
  • Regular and travel within designated area to meet with service providers or resolve serious patient issues is required;
  • Sometimes is exposed to verbal abuse and/or health hazards.
