Job Description

Company BioThe Home and Community Care Support Services South East is responsible for planning, managing and funding health care at the local level, as well as the delivery and coordination of home and community care. One of 14 LHINs established by the Government of Ontario, the Home and Community Care Support Services South East is driven to design and deliver quality, patient-centred health care to achieve the vision of “Better System, Better Care” and meet the needs of residents who call this region home.Committed to ensuring health care services maximize available resources, are aligned with regional and provincial priorities, and meet local needs and priorities, the LHIN continues to work with the Ministry of Health and Long-Term Care, in partnership with patients, families, health service providers and health care innovators to transform the health care system in Ontario – today and into the future.The South East region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smith Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.Position SummaryReporting to the Manager, Home and Community Care; the Placement Facilitator coordinates applications, facilitates and authorizes admissions to long-term care homes for applications to long-stay, short stay and convalescent care from the community, hospitals, and other long-term care homes. The Placement Facilitator also acts as an information resource to internal and external stakeholders.Duties will include, but not be limited to, the following:

  • Receives and monitors information regarding vacancies in long-term care homes within the South East geography
  • Manages wait-lists for South East LHIN long term care homes
  • Maintains bed vacancy for all LTCHs
  • Negotiates with South East LHIN LTCHs around appropriate applicants for the available vacancies.
  • Contacts patient/POA/SMD and conducts bed offer for available beds in South East LHIN
  • Coordinates LTCH Admissions
  • Advises patient/POA/SDM of the financial costs
  • Matches patients to available bed in LTCH, completes bed offers and confirms admissions
  • Authorizes admissions to LTCH
  • Completes “Authorization for Admission” form for the MOHLTC
  • Documents admissions according to policies and procedures
  • Educates and liaises with applicant/POA/SDM in order to achieve a successful LTC admission.
  • Ensures that information regarding vacancies is kept current on the data base
  • Educates and liaises with Care Coordinators to resolve crisis situations
  • Attends and participates in LTC/LHIN meetings
  • Prepares and maintains statistics of admissions to all LTCH’s in South East LHIN.
  • Closes files on an appropriate and timely basis.
  • Educates patients and families on MOHLTC Regulations, Policies, and Procedures.
  • Performs other duties as assigned
Minimum Qualifications & Position Requirements
  • Registered Practical Nurse (RPN)
  • Current registration with the College of Nurses of Ontario as an RPN in the general class
  • Health Care experience in LTC or equivalent LHIN experience is preferred
  • Knowledge of Long Term Care Act, Substitute Decision Act, and Health Care Consent Act
  • Strong data entry skills and ability to work with MS Word, Outlook, Excel and the Internet

  • Ability to work collaboratively with all levels of LHIN staff and external partners
  • Good negotiation, mediation, problem solving and facilitations skills
  • Empathic and sensitive to the needs of others
  • Good organization skills and ability to prioritize effectively
  • Accuracy and attention to detail
  • Competency in the French language an asset
  • Ability to attend work on a regular basis
