That’s why we make every effort as an organization to provide our valued employees a safe, healthy, and rewarding work environment.Reports to: Clinical Team Leader, Southwood HospiceLocation: Southwood Care Centre – 211 Heritage Drive SEType: Full-time, PermanentHours: Flexibility requiredPosition Summary:The Case Manager is responsible and accountable for providing leadership to the care team to ensure the provision of quality care services at the Hospice unit level. Leadership style is flexible and sensitive to the changing needs of the organization, and health care environment. The role is accountable for implementing and evaluating resident care in accordance with the Continuing Care Health Service Standards and Intercare’s Corporate Core Commitments. The Case Manager adheres to Intercare’s policies and procedures in a manner that is consistent with the Employer’s Core Values and in alignment with the understanding that all staff have the ability to make a positive difference.Qualifications/Skills/Job Requirements:

  • Graduate of an accredited School of Nursing with current registration as a Registered Nurse with the College & Association of Registered Nurses of Alberta (CRNA). Bachelor’s Degree in Nursing is preferred; or
  • Graduate of a professional program from a post-secondary institution for nursing, e.g. Licensed Practical Nurse with active registration;
  • A minimum of three (3) years of related care experience, preferably in continuing care - Hospice or palliative experience preferred;
  • A minimum of two (2) years of experience in a leadership role is required;
  • Demonstrated flexibility, organizational and time management skills;
  • Advanced critical thinking, problem solving, decision-making, and conflict resolution skills;
  • Demonstrated ability to find opportunity for improvement;
  • Ability to build trusting, respectful and positive working relationships; and
  • Excellent verbal and written communication and presentation skills with strong attention to detail.
Role Responsibilities:
  • Promote Intercare’s vision of being ‘The Heart of Excellence’ by consistently demonstrating and incorporating the Employer’s Core Values into daily practice and promoting dignity, choice and growth of quality continuing care services;
  • Complete all planning, organization, control, coordination, direction and evaluation of all Resident/Patient Care Services, utilizing ongoing audits, key performance indicators and electronic documentation, while ensuring compliance with all relevant relevant Provincial and Federal Acts and Regulations;
  • Participates in patient admissions;
  • Implements Infection Prevention and Control (IP&C) Mandates, participates in the site Infection Prevention and Control Committee as needed, and conducts high-level audits of practice to assess compliance with IP&C policies, procedures and best practices;
  • Manages outbreaks as per Alberta Health Services (AHS) Outbreak guidelines at the unit level - completes reporting to AHS and the Clinical Program Leader and participates in site-based debriefing following outbreaks;
  • Ensures the provision of high quality resident care, through monitoring and evaluation of the delivery of care on the unit(s) through surveillance and mentoring of front line staff, while mitigating escalating resident and family situations or concerns;
  • Conducts high-level overview of care planning, facilitates interdisciplinary team assessment and care plan development for complex residents, while promoting Resident/Family satisfaction and continuous quality improvement;
  • Acts as a resident advocate and encourages resident involvement in care-related decisions as appropriate;
  • Participates in projects, policy/procedures implementation, risk management programs and committees as applicable, relevant and assigned;
  • Provides Leadership that fosters personal and professional development, demonstrating integrity, loyalty and personal credibility. Demonstrates and promotes ethical standards while participating in and encouraging participation of others in professional activities and development;
  • Facilitates communication within the team through unit meetings on all shifts;
  • In collaboration with the Human Resources department, Clinical Team Leader and Site Administrator, manages the human resource functions of recruitment, workforce planning, employee development, and performance management of employees;
  • Facilitates education within the department’s resident care team through role modeling, ongoing coaching/mentoring and the provision of educational opportunities and ensures staff are knowledgeable about the resident’s rights and responsibilities;
  • Monitors the use of supplies on the assigned unit(s) to promote fiscal responsibility and uses data provided and judgment to determine human and material resource requirements for budget preparation purposes;
  • Participates as a member of the “On Call” team for the facility - hours of work will require flexibility to meet the needs of the unit and facility including scheduled evenings shifts and weekend coverage; and
  • Maintains open communication with key stakeholders, including external agencies;
  • Perform other duties as required.
