Job Description

Community Care Concepts is a non-profit volunteer-based organization that provides high quality support services throughout the Townships of Woolwich, Wellesley and Wilmot that enable seniors and adults with disabilities to remain as independent as possible in their own homes.We are searching for an exceptional individual to work in the community as a Community Support Worker (permanent position working up to 35 hours per week)The Community Support Worker works collaboratively with clients, caregivers, community partners and the staff team to identify and assess client needs and to develop, implement and monitor care plans that address identified needs. Responsibility for coordination of a program portfolio, which is to be defined, will be included in this role. The ideal candidate will possess a diploma or degree in the Social Sciences, Health or a related discipline accompanied by 3-5 years of experience working with older persons or adults with disabilities. Well-developed intake, assessment, referral, crisis assessment, intervention, counselling and case management skills are essential.The position requires a well-developed knowledge base of local communities, community resources and service systems. Computer proficiency as well as strong oral and written communication skills, an ability to work independently, strong problem-solving skills and a knowledge of the needs of the client base are essential for the position. A valid driver’s license and access to a reliable vehicle are required. A mandatory vaccination policy is in place within the organization.Job Types: Permanent, Full-timeSalary: From $34,000.00 per yearBenefits:
