Job Description

Position Summary: The Coordinated Regional Response Team (CRRT) Case Manager will provide guidance, consultation and resources for key organizations, older adults, and their families, who are challenged by complex issues associated with elder abuse. The Case Manager will streamline access to the CRRT, or most appropriate provider and will assist clients in removing barriers when accessing supports. The Case Manager will also support seniors experiencing abuse in acquiring information or documentation for social supports (ie: completing forms, sending correspondence) while building internal capacity of older adults using a compassionate holistic approach.Key Responsibilities: · Work with the Foothills CCR Steering Committee and Older Adult Specialist to increase awareness and understanding of the Coordinated Community Response (CCR) model adopted for the Foothills and Nanton region· Work with Level II/frontline responders to enhance and support collaborative service delivery. Coordinate case management and detailed screening of elder abuse cases reported to the CRRT:o Facilitate CRRT meetings to enhance collaboration and ensure the lawful and appropriate sharing of personal information between partner agencieso Prepare meeting agendas and notify CRRT of elder abuse caseso Maintain meeting minutes and any actions that resulted thereofo Support or implement intervention strategies using a collaborative approach and stay connected with the senior to ensure continuous support.§ Ensure comprehensive support and care for elders experiencing abuse, and ensure follow up is consistently managedo Assist with system navigation, and provide appropriate levels of communication to all partieso Liaise between referral source and CRRT to ensure referral process is complete, relevant case related support network providers are invited to participate, and that all necessary documentation including confidentiality agreements are signed.· Prepare reports for the funding contract from the AEAAN (Alberta Elder Abuse Awareness Network), and communicate same with Foothills and Nanton steering committeeso Develop systems and management of data collectiono Identify and report on gaps in service needs within both regionsEducational Requirements: · Bachelor of Social Work Degree; Diploma in Social Work or other human services fields acceptable with appropriate experience· Current registration with Alberta College of Social Workers requiredExperience and Skills Required: · Minimum 2-3 years’ experience required· A demonstrated ability to work with older adults· Experience providing crisis intervention, working with acute and chronic complexities of Elder Abuse· Demonstrated knowledge of community resources, services and programs available within community and government regarding housing, financial assistance, medical and social support etc.· Current knowledge of all relevant Acts and LegislationCore Competencies: · Flexible and adaptable regarding approaches, supports, and interventions for seniors· Skills in assessment, support, intervention consultation and advocacy are required· Familiarity and working knowledge of issues related to government benefits, housing assistance and employment· Ability to work independently· Skills in working with computerized systems and programs· Ability to exhibit empathy and concern for the problems and needs of the clients· Ability to establish and maintain effective working relationships with clients, associates and other community agencies;Working Conditions: · Works within office setting, during typical day time hours· Travel may be required; valid driver’s license & access to reliable vehicle are requiredReporting Structure: Direct: Director, Clinical Innovation; Indirect: Executive DirectorAbout the Calgary Rural Primary Care Network (CRPCN): The Calgary Rural PCN works together with communities, family doctors and other health team members to do whatever we can help you be as healthy as possible.
