COMMUNITY CARE COORDINATOR (CASUAL) - 4 POSITIONS (NW22-80)NATURE OF THE POSITION: Reporting to the Home and Community Care Manager, this position is responsible for collaborating with patients and their families to develop quality, timely and cost effective individual plans for service provision, based on patient needs, utilizing a multi-disciplinary approach to achieve optimal health outcomes. The purpose of this position is to assist patients in achieving their highest level of functioning and independence, consistent with their values, priorities, capacities and preference of care. The Community Care Coordinator determines patient’s eligibility in accordance with Ministry of Health and HCCSS policies and procedures.QUALIFICATIONS: · A University Degree from a recognized university in a health-related field; an equivalent of education and experience may be considered.· A practitioner in one of the following health disciplines: Nursing (RN), Physiotherapy, Occupational Therapy, Social Work, Dietetics, or Speech-Language Pathology· Current registration in good standing in the relevant Regulated Health Professional College in Ontario· At least two years of relevant work experience an asset· Excellent interpersonal communication skills· An oral/written proficiency with Canada’s two official languages will be looked upon favourably· Must have access to a vehicle and a valid Ontario Driver’s License· Solid ability to use MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.) and internet research skills and computer navigation is required.Job Type: Full-timeSalary: $66,430.00-$85,176.00 per yearBenefits:
