Job Description

Job DescriptionThis is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector that will deliver the next generation of care in Mississauga, providing holistic proactive, seamless care and support to patients, families and caregivers. The Physiotherapist is responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including mobility and function, symptom management, and delivering education.

  • Works under the supervision of the Program Director
  • Collaborates with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of care
  • Participates in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and occupational therapists
  • Conducts physiotherapy assessment, consultation and planned interventions for patients
  • Engages in skilled, compassionate communication with patients, families, and caregivers regarding mobility, and function
  • Educates and mentors patients, family and caregivers on strategies to maintain safe mobility and function in the home setting
  • Maintains accurate and timely documentation of patient assessments, treatment plans, and outcomes, ensuring adherence to ethical and legal standards
