Job Description

Job DescriptionAs part of a tight-knit collaborative team, you will assist palliative patients with personal care and activities of daily living. You will support patients, families and caregivers –enhancing quality of life, comfort, dignity and security in patients’ end of life journey. All direct patient care activities will be performed under the direction of a Neighbourhood Home Care Specialist who is a Registered Nurse or Registered Practical Nurse.

  • Working alongside the interdisciplinary palliative care team, the PSW provides specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures.
  • Provides companionship and comfort, creating a calm and respectful environment tailored to the client #39;s needs, preferences, and goals of care.
  • Assists person(s) with activities of daily living such as bathing, toileting, hygiene, skin care, light meal preparation, feeding and transfers.
  • Performs light housekeeping duties such as washing dishes, taking out the garbage and laundry. Sweeping and vacuuming may be required, if indicated.
  • Listens, observes and communicates any concerns/changes in the patient’s physical and/or emotional condition to the Neighbourhood Home Care Specialist.
  • Travels within assigned region between patients’ residences or facilities.
