Job Description

We encourage underrepresented groups to join our team, be yourself, and help us achieve our vision of for an inclusive workplace and healthcare system.The primary purpose of the Home Care Program is to enable individuals to achieve and/or maintain health and personal independence in the community and to supplement the care and support available from family and friends. This service is provided by a multi-disciplinary team of which the Home Support Worker is an integral member. Duties will include: - working as a member of an interdisciplinary team;- assisting individuals with the activities of daily living including, but not limited to, bathing, dressing, transferring, meal preparation, respite and essential household tasks;- assessing, documenting and reporting client’s progress to Care Coordinator and Home Support Team Leader;- educating, supporting and encouraging individuals and families to take responsibility for meeting their own needs;- maintaining an efficient client schedule of service in coordination with other team members;- fostering a positive working relationship with clients and their families, other service providers and community agencies- participating in case conferences by sharing pertinent information concerning client progress; and,- other related duties.QualificationsMinimum Qualifications: - Applicants must have completed one of the following training courses: Practical Nurse, Resident Care Worker or equivalent training.- The successful candidate must have up-to-date Cardiopulmonary Resuscitation Certificate.- Applicants must have the ability to work effectively with individuals and their families in a community setting.- A valid driver’s license and access to reliable transportation will be required. - The successful applicant must have a good previous work and attendance record. Other Qualifications: - Preference will be given to applicants with recent home care experience.- Experience in related nursing areas (adult with chronic conditions, palliative care, and home care) would be considered an asset.- Training in ISM would be considered an asset.- Additional relevant education and experience would be considered an asset.Important Notes: Notice to all External Applicants - Health Care Workers are required to provide proof of Covid-19 vaccination as per Health PEI’s Covid-19 Immunization and Management Policy. In the event that a new hire is unable to be vaccinated as a result of a medical exemption they will be required to submit supporting documentation to Employee Health / Wellness and Safety.
