Who We Are: Reena is a non-profit organization that has provided community services for people with developmental disabilities while promoting dignity, individuality, independence, personal growth and community inclusion within a framework of Jewish culture and values since 1973. Reena advocates for people, and supports individuals to develop spiritually, feel a connection and sense of belonging to their roots, culture and community, but also encourages the expression of everyone’s culture and religious roots, regardless of background and denomination. The Intake Social Workers main role is to provide information, assessment and entry facilitation into the organizations Community Participation Supports and Residential Systems, and to maintain and act as the contact for the individuals who have chosen Reena to be their lead agency. The position also maintains presence and contribution at a number of external committees to liaise and coordinate services within the Developmental Services Ontario (DSO) scope of authority.1) Intake Responsibilities· Provides initial assessments for individuals seeking or sent to Reena for intake· Conducts meetings with families and other agencies to plan for intake· Prepares all necessary documentation for intake· Produces contracts for individuals entering Reena system· Works collaboratively with all stakeholders and departments as needed, to ensure smooth transition· Acts as resource for inquiries into Reena and DSO services· Provides ongoing case management for families in Reena system· Coordinates initial Individual Support Plan meeting at 3 months following intake into service2) DSO Coordination Responsibilities· Declares and reports vacancies as relevant to the DSO· Attends and contributes at assigned committees or networks to represent Reena· Acts as liaison with other agencies to address inquiries and solutions to intake· Monitors internal vacancies in conjunction with department Manager, Chief Operations Officer and Residential & Day planning committee.· Initiates process of changing needs systemically within Reena and the DSO · Any other responsibilities as assignedQualifications: Formal Education and Experience· Masters of Social Work required· Annual renewal and registration with the OCSWSSW - member in good standing· 2 years’ experience in developmental services· Knowledge of the intake process· Ability to perform service needs assessments· Experience in dealing with families and counsellingCreative Problem Solving, Decision Making and Initiative· Thinks “outside the box” to identify new solutions.· Ability to evaluates alternatives solutions and identifies and acts on the optimum course of action.· Ability to think imaginatively in order to develop creative solutions or options.· Ability to think ahead and plans for contingencies and effectively prepares andorganizes resources and activities.· Ability to manage multiple priorities successfully.· Well organized with keen attention to detail.· Able to prioritize multiple assignments, meet strict deadlines.Interpersonal Skills , Respect and Conflict Resolution· Excellent communication skills, verbal and written· Takes personal responsibility for resolving service issue in a timely manner.· Produces good results when working with others by displaying tact, cooperation,sensitivity and respect for the opinions and situations of others and their culture.Please note all interested candidate will need to comply to the mandatory Vaccination Policy thereafter (i.e. boosters, as applicable); Reena will comply with the Ontario Human Rights Code (OHRC) and accommodate staff, that for bona fide human rights protected reasons are unable to be vaccinated, based on providing supporting documentation that substantiates this requirement.Reena is committed to providing accessible employment practices that are in compliance with the Accessibility for Ontarians with Disabilities Act (‘AODA’).
