Job Description

The Interprofessional Practice Department has an opportunity for aADVANCE PRACTICE LEADER - NURSE, FULL TIME 70 Bi-Weekly Hours| Non-Union | Day ShiftsReporting to the Manager and Interprofessional Practice Chief Nursing, the Advance Practice Leader - Nurse will facilitate clinical care, research, education, leadership, consultation and collaboration utilizing a dynamic approach that is flexible and targeted to the patient, family and organizational needs. The successful candidate will partner with clinical managers to facilitate and maintain accountability for standardized clinical best practice; will work collaboratively with interdisciplinary and leadership teams to develop, execute, facilitate and sustain scholarly nursing practice and exemplary patient care. The Advance Practice nurse will be involved in shaping the strategic direction of the various clinical programs, as well as advocate for scholarly gerontological care with key internal and external stakeholders within the health care system.Responsibilities include but are not limited to:

  • Provides expert knowledge to advance care and meet the needs of residents/patients and families
  • Leads program development and new models of care delivery for the population served, in collaboration with the program leadership team
  • Modeling, encouraging and enabling exemplary care at the bedside
  • Collaborates with program leadership to ensure the implementation and sustainability of exemplary clinical practices
  • Assumes a leadership role within the program(s) and the interdisciplinary team to develop solutions/service delivery models that provide exceptional patient/family care
  • Developing, implementing and evaluating, in a collaborative manner, a sustainable approach to ensure standardized, evidence-based nursing practice across the organization
  • Authoring clinical policies, procedures and medical directives that facilitate evidence-based best practice
  • Encourages and modeling critical clinical inquiry that drives the development of nursing research and improves patient care
  • Contributes to the improvement of clinical outcomes based on best available evidence and efficient utilization of resources
  • Collaborates on patient safety and risk management issues including the development of improvement plans to address shortfall.
  • Acting as clinical resource ‘on call’ on a rotational basis
  • Working collaboratively with other programs and services to ensure quality patient outcomes and effective transitions of patients between programs and care settings
  • Providing clinical consultation to other programs and settings
  • Taking a lead role in the implementation of Required Organizational Practices (‘ROP’) mandated by Accreditation Canada
  • Leading the implementation of best practice guidelines including Registered Nurses Association of Ontario (‘RNAO’) across the organization
  • Working collaboratively with program leaders in the performance review and management process
  • Ensuring individual and organizational accountability standards are met and quality improvement programs are initiated, as necessary
  • Collaborating with program leadership and human resources to recruit, retain and implement performance management strategies
Qualifications include but are not limited to:
  • Master’s degree in Nursing is required
  • Current Certificate of Competence in good standing from the College of Nurses of Ontario (‘CNO’) is required
  • Three (3) years recent related experience in geriatric complex continuing care and/or rehabilitation and expert practice skills required
  • Knowledge in pressure injury prevention and management an asset
  • Experience in adult education/curriculum development and delivery
  • Knowledge of MS Office (Word, Excel and Outlook)
  • Superior communication skills and ability to communicate effectively to direct and motivate team and others, maintaining a persuasive and credible presentation style at all levels of the organization
  • Demonstrated ability to work collaboratively with colleagues at all levels
  • Ability to distill complex information into clear, concise messages
  • Ability to influence others and able to establish credibility with all levels of the organization, challenging the status quo and being a catalyst for positive change
  • Excellent organizational, planning and time management skills
  • Ability to manage multiple, often competing priorities.
  • Demonstrated effective critical thinking, analytical, decision making and problem-resolution skills
  • Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams
