As our Social Worker I with the Complex Care Support Team, you will be a member of a multidisciplinary team and provide a range of social work services to patients and their families in their homes, designated supportive living and long-term care. The Complex Care Support Team (CCST) Social Worker provides face to face and virtual consultation services for the North Zone clinical teams in their care for residents and clients, when clients have complexities that influence their care and wellness. This position is located in Valleyview, with travel required, and you could be eligible to receive one of our northern allowances. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.
