Job Description

Visiting Physician/Medical DoctorSILVERADO CREEK SENIORS COMMUNITY7 Silverton Way SW, CalgarySilverado Creek Seniors Community “Silverado” is proud to delivery high quality Long-Term Care services to our Residents.We are looking to Partner with a Visiting Physician/Medical Doctor to provide Medical Care to approximately 15 residents of Silverado.In your role as Visiting Physician, you work along side the Medical Director, Director of Care and an interdisciplinary team to support the medical practice in the home.Specifically, we ask for the following;

  • Delivery of Medical Service
  • Assume the role of Most Responsible Practitioner for provision of medical care to all assigned residents.
  • Visit the Care Centre regularly to review the Physician Communication Binder and for resident rounds and to encourage collaborative care and interaction with interprofessional staff, routine rounds should occur during regular working hours.
  • Visit each resident at least once a month.
  • Document all medical care provided in writing on the Electronic Health Record and/or paper chart according to the Care Centre’s documentation practice
    • Review and sign verbal orders on the next visit and at least within thirty days (30).
    • Provide a clinical indication for use for all medications ordered.
    • Review and initial all lab, X-ray and other reports.
    • Review consultations and recommendations made by care providers involved in the care of the resident.
  • Complete a written medical assessment within seven (7) days of the Resident’s admission to the Care Centre and update annually as required.
  • Collaborate with team members in performing Medication Reconciliation at admission, and participate in periodic collaborative Medication Review when required, with a focus on appropriate prescribing in elderly or LTC patients and reducing anti-psychotic use and polypharmacy.
  • Work in partnership with interprofessional staff to develop a comprehensive Care Plan for the management of complex multimorbidity in the LTC patient. Participate in collaborative monthly review of all residents who are using a chemical restraint/antipsychotic medication to assure appropriate use.
  • Provide medication, diet and treatment orders upon notification of the Resident’s admission.
  • Complete annual reviews at care conferences.
  • Participate in Advance Care Planning discussions with the Resident and Substitute Decision Maker (SDM) and provide Goals of Care Designation Order at admission.
  • Review Goals of Care Designation Order during the admission conference, annual conferences and at the time of significant change in Resident’s health status.
  • Participate in Initial and Annual Care Conference.
Regulator Requirements.
  • Be licensed to practice in the Province of Alberta and be in good standing with the College of Physicians and Surgeons of Alberta.
  • Have required AHS Medical Staff privileges granted by the Department of Family Medicine to practice in the Long-Term Care Centre, maintain appropriate privileges and participate in Periodic Reviews.
  • Notify the Medical Director of practice restrictions from the College of Physicians and Surgeons of Alberta or other changes affecting ability to practice.
  • Comply with the Alberta Health Services Medical Staff Bylaws, Alberta Health Services Medical Staff Rules, Alberta Health Services Continuing Care Service Standards, and local policies and procedures pertaining to the provision of medical services in the Long-Term Care Centre.
