Job Description

Applications are currently being accepted for a full-time Facilitator in the Supported Independent Living (SIL) program.Job Purpose:To provide personal support exclusively to one (1) adult with a developmental disability in a supported individual living setting. The objective is to implement an Individual Support Plan for continued independent living & client enjoyment of a lifestyle which promotes participation, friendships & citizenship in the community.Job Responsibilities:To coordinate the case, intake & assessmentTo develop, implement & provide services based on & reflective of person-centred planning, self-determination & choiceTo ensure that the client receives the contracted hours of support as identifiedTo develop & maintain a trusting & empowering relationship with the clientTo facilitate the continued development of skills necessary to live independently & safelyTo ensure safe & suitable living arrangementsTo plan for the client’s changing needs as a result of aging, medical, &/or psycho-social needsTo develop opportunities for increased inclusion within the client’s community, as well as creating opportunities for increased friendships & affiliationsTo provide the client & caregiver(s) with community information & service referralsTo facilitate crisis intervention for the client (abuse, post-traumatic stress, panic situations, suicidal ideation or other critical incidents), advocate & support the client or refer the client to other resourcesTo ensure the client accesses appointments & services when requiredTo meet all service delivery standardsTo maintain case files with historical & current information regarding the client’s status, assessment, service plan, service provided, outcome & evaluation, & all case notesTo provide statistical & incident information to manager for monthly & quarterly reportsTo write letters or memos regarding serviceTo participate in providing information, & contributing to special reports, proposals or projectsTo attend all required training: e.g. Social Role Valorization, Personal Outcome Measures, First Aid & CPR, CPI, & other training as relevantTo comply with current Ministry serious occurrence reporting & guidelinesTo participate in team meetings & provide input & ideas for improving services.To contribute to team assignments & projectsTo maintain confidentiality for all matters related to Agency work in accordance with relevant Agency policies & proceduresTo participate in orientation & training of placement studentsTo perform other relevant duties as assigned, for team or AgencyKey Qualifications:Bachelors level degree in related field &/or community college diploma specific to developmental disabilitiesExperience requiredDemonstrated experience in working with people with a dual diagnosis (complex mental health needs & developmental disability)Current 1st Aid & CPR certificationSuperior written & oral command of the English languageStrong communication skills, including presentationsComputer proficiency in MS Word, Excel, & data basesAbility to work from home, consisting of a safe & private home office setting & the necessary technical equipment & high-speed Internet access for remote workFamiliarity with virtual work platformsDemonstrated efforts to continuously update skillsAbility to work day, evening & week-end hoursA valid Ontario Driver’s License in good standing, a clean driving record, appropriate insurance & a vehicle are required for this position.Flexibility to work at a variety of locations throughout Peel regionAdditional language(s) an assetWe welcome applications from all qualified persons.
