If you are passionate about what you do, motivated to improve the health of the community, committed to excellence, quality and patient safety we would like you to join our Better Together team.As part of the Flow, Operations and Patient Registration Program, and reporting to the Manager, Patient Flow, the Patient Care Coordinator (PCC) is a clinical liaison who provides operational leadership to hospital staff on evening, nights, and weekends, and is responsible to proactively facilitate flow from a hospital –wide perspective with a particular focus on reducing ED length of stay (LOS) for admitted patients.Position Summary:The Patient Care Coordinator (PCC) will work within a fast paced environment as a reliable, collaborative interdisciplinary team member.In addition to being one of the key points of contact for the organization after-hours and during weekends/stat holidays, the PCC is responsible to:

  • Develop a flexible, responsive system to meet changing and competing demands
  • Support emergency codes and other unplanned situations
  • Conduct regular rounds on all nursing units to maintain visibility, continually tracks and determines alternate options for bed placements to mitigate congestion in ED and other admitting sources
  • Provide co-ordination of patient admissions, discharges and transfers utilizing THP’s flow and access tools, standards, protocols and current awareness of each units status within the hospital
  • Track use of over census beds
  • Develop and maintain strong, effective liaison with member of the health care team in order to facilitate discharge and admission planning
  • Assess and make recommendations where process is not effective in facilitating flow
  • Act as an onsite resource for capacity and flow, to provide leadership and support to Coordinating nurses, Clinical staff, Bed Allocation staff, Physicians and Patients/Families.
  • Liaises and escalates pertinent issues to Clinical Operations Manager or Senior Leader on-call, where appropriate
  • Ensures follow-up and communication with direct report (Flow Manager) on any relevant issue(s)
  • The primary goal of the Patient Care Coordinator is to facilitate and enhance patient flow organizationally and to mobilize teams to achieve throughput. The PCC does this with the goal of providing an exceptional experience for patients, families and staff. The PCC understands the demand and capacity challenges of a hospital system and works to achieve patients being in the right place, at the right time, receiving the right care.
Key Qualifications:
  • Undergraduate degree in a health care discipline (nursing preferred)
  • Current member and in good standing with a professional college required
  • Minimum 3 years recent clinical experience in an acute care hospital setting
  • Proven leadership skills
  • Strong facilitation and conflict resolution skills to help resolve challenging situations
  • Ability to think critically to respond to a rapidly changing environment
  • Ability to assess risk, and effectively utilize resources to support patient acuity while considering the global needs of the organization
  • Ability to be self-directed and thrive in a dynamic environment
  • Role models composure; Able to remain calm under pressure
  • Demonstrates positive, collaborative working relationships with interdisciplinary health care teams
  • Excellent customer service skills
  • Ability to listen, engage, collaborate, negotiate, facilitate and entertain creative solutions in order to enhance patient flow organizationally and mobilize teams to achieve throughput
  • Actively participates in and demonstrates support for continuous quality improvement initiatives
  • Excellent verbal and written communication skills
  • Positive work and attendance performance essential
