If you are passionate about what you do, motivated to improve the health of the community, committed to excellence, quality and patient safety we would like you to join our Better Together team!Position Summary:At Trillium Health Partners the Mental Health programs provide psychiatric assessment, short-term treatment and stabilization for adults and seniors. The program areas are staffed by a multidisciplinary team that includes psychiatrists, nurses, social workers, occupational therapists, and pharmacists. Treatment modalities include nursing care, group activities, pharmacological treatments and individual therapy.Reporting to the Patient Care Manager, the peer support worker will work as a member the multi-disciplinary team to provide support to patients admitted to the Hospital, and within the Early Psychosis Intervention Program. The Peer Support Worker works with patients in individual and group settings to facilitate and support recovery for adults and youth facing mental health challenges through empowerment, compassion, empathy and mutual understanding. The Peer Support Worker has gone through the process of their own recovery and they will use this experience to assist others on their recovery journey.Responsibilities:

  • Supporting peers and family members within the mental health program
  • Advocating on behalf of individuals and participating in service resolution
  • Building and maintaining linkages to service providers throughout Mississauga, Halton and West Toronto
  • Assist with intake and assessments and safety plans
  • Assist in navigating mental health services
  • Provide mentorship role
  • Provide support to complete referrals, accompany clients to appointments as needed
  • Complete necessary documentation according to hospital guidelines
  • Participate in assigned seminars, training and education sessions and in regional Peer Mentor advocacy groups and best practice development
Qualifications:
  • Must have lived experience within the Addictions and/or Mental Health system
  • Must have WRAP Level One; WRAP Level Two is an asset
  • Knowledge of principles of early psychosis recovery and of the NAVIGATE Model
  • Knowledge of psychosocial rehabilitation and Recovery Oriented Practices
  • Have or be willing to obtain Core Skills Training, Peer Worker training, ASIST, Mental Health First Aid, and First Aid & CPR
  • Excellent customer service skills and knowledge of common challenges faced by those navigating this system
  • Satisfactory employment and attendance record required.
  • Proven confidence to advocate with an open mind of lived experience, within the addiction and mental health system to reduce stigma
  • Awareness of Ontario Disability Supports Program(ODSP), Ontario Works(OW), Health and Safety, Ontario Human Rights, Employment Standard Act
  • Demonstrated ability to work within a team environment and independently
  • Ability to adapt to various work environments and able to flex work hours to meet program, employer and client demands
  • Completed post-secondary education in a related field or equivalent work experience may be considered
  • Active in your own recovery process, self- awareness, ability to draw on personal experiences to engage, validate and role model when appropriate.
