Job Description

Location / Lieu: Ottawa / East RegionPosition / Poste: Case Manager – Bilingual (English / French) preferredDuties / Fonctions:

  • Assist with the administration, coordination, education and financial / statistical monitoring of the Passport Program funded by the Ministry of Children’s, Community and Social Services
  • Liaise with service coordinators and Developmental Services Ontario to review options for individuals / families
  • for community participation supports and / or caregiver respite and monitor any service gaps
  • Manage large numbers of case files with extensive financial follow up and tracking progress
  • Assess individual needs as they relate to the mandate of the program
  • Communicate approved funding levels to individuals and / or families following prioritization process
  • Provide case management to individuals and / or families who present with complex disability and service needs
  • Provide individuals and / or caregivers with community resource options for accessing appropriate services
  • Respond to concerns and / or complaints regarding particular situations with individuals, families and /or community stakeholders
  • Significant participation required in attending community activities (including presentations, trade shows and service fairs) educating stakeholders on the Passport Program
Qualifications / Compétences requises:
  • Knowledge of the Passport Program

  • Relevant Bachelor’s Degree or College level Diploma in Social Services field
  • Minimum 4 years of direct experience working within the field of Developmental Services
  • Bilingual (English / French) PREFERRED
  • Knowledge of the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act
  • Knowledge of the available community services and supports throughout the Eastern Region - includes Ottawa English and French communities; Renfrew County; Stormont Dundas & Glengarry; and Prescott Russell
  • Sensitivity to the issues and barriers that individuals with Developmental Disabilities and their families experience
  • Superior organizational, administrative and time management skills
  • Exceptional problem solving ability and demonstrated sound judgment skills
  • Able to work independently and as part of a functional team
  • Exercised ability to work in a fast paced environment and handle a number of tasks simultaneously
  • Must have a valid driver's license and access to reliable vehicle for travel within Eastern Region
  • Excellent computer skills in word processing, excel and database management
