To help fulfill its mission, LOFT is hiring a Psychogeriatric Case Manager.Job SummaryAs a Psychogeriatric Case Manager, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. The role will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.Crisis Outreach Services for Seniors (COSS) is an on-call mobile crisis service for older adults (65+ years) who are frail, socially isolated, at-risk of repeated hospital use and/or elder abuse due to living with mental health and substance use challenges. The COSS team delivers services within 24-48 hours either in the individuals own home or in their local community. Short-term crisis response is available 365 days a year. The team provides crisis counselling, short-term intensive case management, harm reduction, mental health and addictions support, health assessment and care as need by a Nurse Practitioner, and referral to primary care. COSS is a partnership with Woodgreen and Reconnect ensuring that all of Toronto has access to this service.LOFT’s COSSteam supports those individuals living within the northern Toronto geographical area.Note: Staff will be required to work in client’s own homes and may therefore be exposed to second hand smoke. Occasional evening and weekend hours may be required.Frequent travel will be required using own vehicle.What You Will Do:
