Job DescriptionUnion: Non-UnionSite: Toronto Western Hospital Department: Allied Health, Outpatient Cardiovascular Prevention and Rehab Program and Cardiology ClinicWork Model: HybridReports to: Manager, Allied Health Grade: D0:08Hours: 37.5 hours per week Salary: $40.98 - $51.22 per hourShifts: 8.00am - 4.00pm - WeekdaysStatus: Temporary Full Time (approx. 12 months)Closing Date: Until FilledPosition SummaryAs an integral member of the inter professional team in the Outpatient Cardiovascular Prevention and Rehabilitation Program and Cardiology Clinic at the Toronto Western Hospital site, and a specialist in the delivery of nutrition therapy in cardiovascular health, the Registered Dietitian (RD) provides services based on standards of quality and evidence-based practice.Duties

  • Provides consultation and independently screens patients for nutritional intervention, collects and interprets patient health data for in-depth assessment, completes ongoing monitoring and reassessment in collaboration with patients, caregivers, and the health care team
  • Through patient-centered approaches, the RD establishes priorities, strategies and interventions for individualized treatment plans that allow the patient to achieve optimal outcomes
  • As an interprofessional team member, the RD fosters a culture of safety by minimizing patient harm, promoting the well-being of patients and health care providers, reducing the likelihood of adverse events, and communicating safety concerns
  • Implementing pertinent medical directives, completing administrative responsibilities, supervising assigned personnel, conducting practice-based research and performing other functions consistent with the job classification to ensure excellence in the provision of care, if required
