Where else will you get to create a personal wellness plan as part of your performance goalsPosition SummaryThe Regional Mental Health and Wellness Liaison plays an integral part in advancing FNHA’s Vision of “Healthy, Vibrant and Self-determining BC First Nations Children, Families and Communities” in collaboration with the regional mental health and substance use care system. This position works with individuals, families, communities and regional health authority mental health and substance use teams, including specialized care and support programs, to improve and ensure linkages between regional health authorities and services provided by FNHA and communities. As well, the position provides expert level navigation on the availability of mental health and wellness services and supports across the region to individuals, families and communities.As a health and wellness organization, the First Nations Health Authority has aligned with the Provincial Public Health Order released on October 14, 2021. We have a mandatory COVID-19 vaccination policy for all employees at First Nations Health Authority. This policy requires all employees to be fully vaccinated against COVID-19. If selected for employment, you will be required to provide proof of vaccination by providing a copy of your BC Vaccination Card Passport with the QR code.Responsibilities

  • Develops and maintains partnerships with regional mental health and substance use teams and community health service providers to collaboratively support individuals and families to navigate the mental health and substance use care system
  • Develop, recommend and implement practices and procedures to improve access to and continuity of mental health and wellness care
  • Acts as an advocate for individuals and families to improve transitions within the broader mental health and substance use system and ensure linkages are made that support ongoing wellness
  • Facilitates continuity of care by building collaborative relationships with and maintaining current knowledge of local and regional clinical and traditional supports to ensure cultural safety and consistent access to service.
  • Follow up on discharge planning and/or create care plans that support individuals and families to connect to comprehensive, culturally safe, trauma-informed mental health and wellness supports at home and away from home
  • Collaborate with regional and provincial FNHA mental health and wellness services and community and Nation based clinical and cultural support services to ensure timely access to a comprehensive range of supports that promotes healing

  • Bachelor’s degree in social work, human services or acceptable combination of education and experience
  • Significant, recent and relevant mental health and wellness experience (usually acquired over a period of 5-7 years)
  • Experience working in a regional mental health and substance use setting and in particular working with First Nations individuals, families and communities or First Nations serving organizations
  • Experience working with multi-disciplinary teams specific to mental health and wellness
Knowledge, Skills & Attributes
  • In-depth knowledge of the BC mental health and substance use system and the current landscape of First Nations health in BC
  • Solid understanding of the First Nations Perspective on Health and Wellness
  • Wide, regional knowledge base of existing mental health and substance use services and supports
  • Thorough understanding of person centered principles to provide holistic, integrated, coordinated, and high-quality care that respects diversity and culture
  • Trauma informed practice
  • Physical ability to perform the duties of the position
